Years and years ago there used to be an organization called Data Entry
Managers Association(f/k/a/ Keypunch supervisors).  If they are still
in
existence, I'm almost positive that the standards you want were their
way of
life.  Otherwise, you might see if your IT people have any sort of
standards.
