There was a question several weeks ago regarding reporting HIPAA violations.   I received some information on reporting violations from the Office of HIPAA Standards, Centers for Medicare & Medicaid Services (CMS) and am passing this information on to those who may need it.

Privacy violations should be reported to the Office of Civil Rights in HHS
All other HIPAA violations should be reported to CMS.  There is a complaint form on our web site (www.cms.hhs.gov/hipaa)
