RE: SUMMER HEALTH INSURANCE SUBSIDY PLAN DEADLINE
If graduate students on appointment want to participate in the health
insurance summer subsidy plan, they must maintain coverage from 15 May 2006
through 14 August 2006.
Students who request a shorter plan period must submit the UF International
Center's health insurance waiver form, documenting that they will be out of
the country during the weeks or months for which they have not bought
coverage.
Coverage for this semester must be bought by 3 February 2006 for subsidy
payments to be captured through payroll deduction. Failure to register for
payroll deduction by this date will make graduate students ineligible for
the summer subsidy plan.
