Thanks for the hints.  Unfortunately due to firewall and business 
restrictions at my work, I'm not allowed to have yahoo accounts or upload 
files to non-project related sites.  I did join the first yahoo group 
through my home e-mail account.  I got about 10 requests for the form, so 
I ended up just sending them out to each as they sent me an e-mail.  The 
only one I couldn't send was to one request from Canada which I will have 
to do through home.
