I think that the photography folks at University Relations might have
the sort of thing you are looking for.
The pictures that are part of their stock library (photos.urel.ufl.edu)
are already covered by the sort of paperwork you are looking for.

Subject: permission to post people photos
Does anyone have a preferably-already-vetted-by-some-UF-lawyer form for
getting permission to post photos of people on the web? I'm not talking
about things like the faculty page where the individual provides a
shot. I'm talking about lab or classroom pictures etc. that include
students. Is this required if the individuals are not identified?
