Hi Gergei,

You can try to use LimeSurvey (http://www.limesurvey.org) - open source
software. It's a general purpose survey tool that can be used to collect
the SN-type data too.

It's very flexible and has an advanced user management feature. However,
we discovered that it can't properly handle a large network of over 100
people.
