Dear Gergei,
we also used LimeSurvey. To get an impression you can have a look at a
template here:
https://survey.ivt.ethz.ch/soziale-kontakte/template/english/WelcomeEng.html
We implemented google-maps to get the coordinates of e.g. home
locations, which was done with java-script. Due to different browsers we
had some problems with these scripts.
Because of that we developed a new form in java. Unfortunately it is
only available in German yet. Please let me know if you are interested.
