We do a survey on all of our tutoring staff following each session. It's anonymous and electronic. Students can complete it in our Reception area on their way out of our central facility, or our tutors in remote locations turn the laptop toward the student (out of the tutor's view) and let the student complete it. The survey takes less than 90 seconds to complete in most cases and assesses the student's satisfaction levels with the tutor's working knowledge, the specific session, the department holistically, whether or not they would recommend the tutor and (separately) the CLR to another student, etc. There are fields for additional feedback.
We didn't see great response rates in prior years with paper surveys and/or end-of-the-semester surveys, so we did this one via LimeSurvey (also tallies data and can be purchased for a reasonable price through your IT department) starting 1.5 years ago with a promotion attached. Each time a student completes the survey after a tutoring/SI/workshop session, they receive a stamp on a card. Six stamps = entrance into a giveaway for an iPad Mini (1st prize) or Dunkin Donuts $25 gift card (2nd prize) at the end of the fall and spring terms. (We chose six visits based upon research out of the CCCC that six visits for a student struggling with a course, with the first occurring before midterm, = meaningful course progress. We don't require that all six visits be for a course or in any timeframe, but we do mention this statistic in our pitches in the classroom and at events.)
Our UG and G student populations, FT and PT, are about 6,500-6,800 annually. The first year we did this promotion, we supported about 24,000 student visits, and we saw >1,750 surveys completed. If you buy the iPad Minis at a discounted rate, the cost is worthwhile (giving away 2 a year) for the amount of data and feedback collected. Most of it was very positive, but I was able to hone in on concerns in relation to a few tutors and follow up with them.

Our evaluations are very similar to these and we also send one mid semester and at the end of the semester. The one comment I want to add is that we send them via a Google Form. (We collect emails from all our participants.) This allows us to easily read and report on the data. It is not anonymous but that doesn't seem to be an issue. We have found offering a small
($20.00) incentive (gift card to our bookstore) greatly increases participation!
