You may want to look at Pieceing It Together:  A Guide to Academic Success
by Lisa D'Adamo-Weinstein and Jane Jenson.   In1998, Lisa led the  MCLCA
post-conference institute entitled "Laying New Foundations for Success:
Self-Reflection and Focused Inquiry for Students in Academic Difficulty."
Her presentation was excellent, and the parts of the book I saw were very
helpful.
