Hi Danielle,

A simple online survey tool is SurveyMonkey (http://www.surveymonkey.com).
If you want to control the survey and store it on your server and be able to
backup and restore (and also are technically inclined), then you want to use
something like PhpSurveyor (http://www.phpsurveyor.org).
