I know that you are seeking a formal test anxiety inventory, but here is an 
informal, self-report one that I use in my study strategies course. It is 
located at the web site for Wright State University.

I like this inventory because it measures both Debilitating anxiety and 
Facilitating anxiety. It is a good opportunity to discuss with students the 
importance of test preparation and the fact that not all anxiety is bad.
