At University of Maryland our Learning Assistance Service keeps student
records (intake form, problem checklist, etc.) for 5 years.  We have an
access database where demographic data, service use, etc. are kept
indefinitely.  We just finished a massive shredding project here in our
whole counseling center and came up with a policy for destruction of
records.

Hello.  We are currently have a large amount of paper documentation for
students who have received services through our center, primarily
placement testing and tutoring.  We are interested in the length of time
other institutions retain records of correspondence, copies of student
test results, tutorial request forms, etc.

Also, is anyone using ImageNow software to keep track of that
information electronically?
