Gavilan College is in the process of developing instruments to measure out
PLO's.  I have been piloting a couple of things for the Tutoring Center.  I
have students specify their tutoring objective on a Request for Tutoring
Form (attachment) and then replicate the same list of objectives on a
participant survey (attachment) that I send out at the end of each semester.
I also use a tutoring session evaluation form (collected for a couple of
days mid-semester) see attachment.  Looking forward to seeing more
examples--I'm still working on this.

Dear Learning Assistance Colleagues,



I am in the process of developing an assessment plan for my area, and
would like to know if anyone has examples of direct outcomes
measurements related to tutoring, and other academic success initiatives
(e.g., academic consultations, workshops, etc.). I've developed learning
outcomes for the First-Year Seminar course on my campus, but am having a
more challenging time doing the same with some of the other programs in
my department.



Any advice or examples would be greatly appreciated.
