Good afternoon,

Has anyone done any sort of customer service reviews or surveys for your department, i.e., requesting feedback from your customers on how well (or not well) you're doing?  If so, would you mind contacting me off list as I'd like to speak with you.  We're looking at doing something similar and are not sure what kinds of questions to ask; what format the survey/assessment should be in, etc..
