I've attached the tutor evaluation we use. It is adapted from Martha
Maxwell's "Improving Student Learning Skills". I've added a question at
the bottom so that I can measure the tutor's Net Promoter Score (NPS).
The NPS is the percentage of respondents giving the tutor a 9 or 10 (the
promoters) minus the percentage giving them a 1, 2, or three (the
detractors). This measure is everywhere in the business world.

Chris:  I use a student response form that the student fills out at the
end of every one-on-one session; it gives me a fairly good reading of
how the student and the tutor interacted and where potential problems
might lie.
