We've been using Limesurvey (http://www.limesurvey.org ). It's open source 
and you can modify the templates and/or code yourself if desired or use as 
is. It's been especially useful for creating pair-comparison measurement 
questions as well as incorporating images and sound. The developers are 
great and help is available in both the manuals as well as the forums 
(http://www.limesurvey.org/en/forum ). 
You can also use Limesurvey to easily create non-english surveys (they have 
documentation and forums for non-english languages too). We've used it most 
often for surveys in English but have run some in Chinese. And once someone 
from the German forum responded to an obscure question I had re: sound.
very satisfied. Can also do all the regular survey things (conditional 
questions, token invite emails, etc.)

I've been investigating Network Genie (https://secure.networkgenie.com/) 
and ONA Surveys (http://www.onasurveys.com/) and wonder what other social 
network online survey options are out there. 

If you would let me know what other sites you've used and your satisfaction 
with them, I'd greatly appreciate it.
