Attached is the survey we used in our Social Networks and Networking study that has an ego-centric module that worked well.

These data were the basis of our paper:

Valente, T. W., Fujimoto, K., Soto, D., Meeker, D. & Unger, J. B. (2013).  Variations in network boundary and type: A study of adolescent peer influences. Social Networks, 35, 309-316.

b)      On-line software for implementing ego-network surveys, that can accept Arabic script.
