Hi Andrew, 

If you have a predefined list, I believe ONAsurveys is you best bet. Their
functionality is very network-research friendly. 

The market-research oriented web survey services are not as network-research
friendly. Qualtrics, Survey Monkey, Survey Gizmo, and all other major Web
survey services probably have the carry-forward feature as Michelle
mentioned. Note that each company has a slightly different way to call it,
but you can definitely use that to insert the names from previous responses
into the next question. The biggest problem is, NONE of these major web
survey service can help you with alter-alter relationship question. You can
and will find a solution with very ugly layout and poor usability and  you
will suffer the methodological issue identified by Matzat & Snijders (2010).

Tobias Stark has developed a graphic module to collect ego network data, but
you need to reach out to him and see if he can share his survey tool. 

If you have computer programing experience, you could give egoweb 2.0 a try.
Or you can check out my Egogalaxy. 

Finally, regardless of what you choose to use, be sure to read Matzat &
Snijders (2010) and Pustejovsky & Spillane (2009) articles in Social
Networks. You need to have a good idea of the methodological challenges in
order to make an informed decision. If you want to read more, you can find a
