I am coming in late on the thread about sharing the names of students who have used tutoring services.  I run a writing center and while we don't inform faculty that their students have used the writing center we let the students tell them in a way. It's a simple, easy system: years ago I had a writing center stamp made (the kind that is self-inking and is hard rubber). The stamp has the name of the writing center  has the name of  the writing center and we use the stamp to let professors know the student came to the writing center and worked with a tutor. The only time we use it is when the student requests proof of attendance and the tutor stamps the student's paper only after a session. We don't use the stamp often, but students like it, we don't have FERPA issues, and it doesn't happen often.
