Howdy all, the Academic Achievement Center would like to capture information from students about why they are coming to use our tutorial services, what services they might benefit from, and how they learned of/were referred to us. We want
this for our own evaluation work, to improve branding/outreach, and to improve coordination with other campus support centers.
Does anyone out there already use an “intake form” that seeks this kind of information from students? If so, do you have students complete it when they schedule an appointment or have tutors complete it with students during their first
tutoring session? Conversely, do you have students provide this info through an automatically generated survey after a tutoring session?
