It’s good that you twitch but I feel Jered is on the correct track.

With FERPA, the student can sign off using a form developed by your institution/office. Check with the institutional research folks or the provost’s office.

All that said, it might be just as simple to develop a referral process whereby the instructor can complete a more generalized form of support request for the student. If there are specific content items, that can be noted and you are not getting the actual test. We proctored tests, as well as had practice tests for our tutors to use but all of that came through the faculty with their support. It’s the best way to help keep everyone accountable with little finger pointing.
